Frequently Asked Questions
What is Form I-693?
Form I-693 is the medical exam form required for many immigration applicants. A USCIS-designated civil surgeon completes it to confirm the applicant does not have any health issues that would make them inadmissible to the United States.
How much does Form I-693 cost?
It’s a medical certification that helps a patient request an exemption from the English and/or civics portions of the citizenship test. It must be completed by a qualified medical professional who can document the patient’s condition.
What is the purpose of the I-693?
The purpose is to show that the applicant has received required vaccinations and does not have certain communicable diseases. USCIS uses it to make sure the applicant meets the health requirements for adjusting status or completing an immigration benefit.
How long does it take to get a green card after the medical exam?
It varies widely. Some applicants see movement within a few weeks, while others wait several months. The medical exam is only one piece of the process, so timing depends on the applicant’s case, background checks, and USCIS workload.
What can I expect at the I-693 medical exam?
The exam usually includes a basic physical, review of medical history, required vaccinations, and tests for conditions like tuberculosis. The civil surgeon completes the official I-693 form and seals it in an envelope for USCIS.
How long is the I-693 valid for?
Form I-693 is generally valid for two years from the date the civil surgeon signs it, as long as USCIS receives it within 60 days of that signature.
How much does the I-693 civil surgeon exam cost?
Costs vary by provider. Most civil surgeons charge somewhere between $200 and $500, depending on tests, vaccines, and location.
How much do doctors charge for I-693?
Each office sets its own pricing. The fee often includes the exam, required tests, and completing the form, but prices can differ widely.
Is a civil surgeon exam covered by insurance?
Most insurance plans do not cover immigration medical exams. Some offices will bill insurance for vaccinations, but the exam itself is typically self-pay.
Do you have to pay for the USCIS medical exam?
Yes. The applicant pays the civil surgeon for the exam and related services. USCIS does not charge a fee for the form itself.
Can I use my own doctor for the USCIS medical exam?
No. Only a USCIS-designated civil surgeon can perform the I-693 exam and complete the form.
How long is my I-693 good for?
It is valid for two years, as long as USCIS receives it within 60 days of the civil surgeon’s signature.
How much does a permanent green card cost?
The government filing fee for a permanent green card depends on the type of application. The medical exam cost is separate and paid directly to the civil surgeon.
What vaccines are required for I-693?
USCIS requires certain vaccines based on CDC guidelines. These often include measles, mumps, rubella, varicella, Tdap, influenza, and others based on age and medical history.
What are the required vaccines for the I-693 form?
Required vaccines vary by age but commonly include MMR, varicella, Tdap, influenza (in flu season), and hepatitis B for certain age groups. The civil surgeon reviews what’s needed for each applicant.
What vaccines are mandatory for a green card?
Vaccines recommended by the CDC’s Advisory Committee on Immunization Practices (ACIP) for the applicant’s age group are mandatory. The civil surgeon determines which ones apply.
What documents do I need for the I-693 exam?
The applicant should bring a valid ID, any vaccine records, medical history information, and any prior test results if available.
Which vaccines are absolutely necessary?
The civil surgeon follows CDC requirements. If a required vaccine is age-appropriate and medically safe for the applicant, it must be given unless there is a medical contraindication.
What happens if you don’t have immunization records for a green card?
If records are missing, the civil surgeon may give needed vaccines, order blood tests to check immunity, or document a medical reason why a vaccine should not be given.
What may cause you to fail an immigration medical exam?
Conditions of public health concern such as untreated tuberculosis, certain infectious diseases, refusal of required vaccines without a valid exemption, or failure to complete required testing can delay or affect approval.
Where can I get Form I-693?
The form is available for free on the USCIS website. Applicants should not fill out the medical sections; the civil surgeon completes those.
How much does the I-693 exam cost?
Costs generally range from $200 to $500 depending on the office, vaccines needed, and testing.
Can any doctor fill out I-693?
No. Only a USCIS-designated civil surgeon can perform the exam and complete the form.
Can any doctor do an immigration medical exam?
Only civil surgeons authorized by USCIS can conduct the exam in the United States. Panel physicians abroad complete a different version for consular processing.
Can a green card be approved without a medical exam?
In most cases, no. A completed I-693 is required before USCIS approves a green card application. There are very limited exceptions.
Is I-693 covered by insurance?
Usually not. Some insurance plans may cover vaccines, but the exam fee itself is almost always self-pay.
