Our integrated medical approach is ideally suited to meet the health care needs of you and each generation of your whole family. Our primary care physicians are highly trained in treating medical conditions involving any organ system in persons of all ages, male and female – we offer your entire family a “Medical Home.”

Our practice provides superior quality primary care for adults and children and was certified by MN Department of Health as a Health Care Home in 2013.

Our comprehensive, person-centered care model is known as the medical home.

A medical home is not a building, house, or home health care service. Rather, it is an approach to providing quality, comprehensive care for children, youth and adults. Through this model, our team of professionals works together to help you and your family coordinate, access and understand care options that are important for your overall health.

We treat patients with respect, dignity, and compassion.

Through the medical home model, we work in partnership to build and enable strong, trusting relationships among patients, families, providers and staff. You will always be able to schedule an appointment with your preferred doctor, and we will strive to meet your specific personal and cultural needs. Primary care is available in English, Spanish and Russian. Our doctors are also well-versed in treating patients with interpreters present, and take extra time to ensure non-English speaking patients fully understand everything that occurs during an appointment.

Patient-Centered: A partnership among practitioners, patients, and their families ensures that decisions respect patients’ wants, needs, and preferences, and that patients have the education and support they need to make decisions and participate in their own care.

Comprehensive: A team of care providers is wholly accountable for a patient’s physical and mental health care needs, including prevention and wellness, acute care, and chronic care.

Coordinated: Care is organized across all elements of the broader health care system, including specialty care, hospitals, home health care, community services and supports.

Accessible: Our patients are able to access services with shorter waiting times, “after hours” care, 24/7 electronic or telephone access, and strong communication through health IT innovations.
